DISTINGUISHED CIVILIAN HUMANITARIAN AWARD

  1. 1. THE FOLLOWING INFORMATION IS GUIDANCE ON THE JOINT SERVICE AWARD AND AN IMMEDIATE CALL FOR NOMINATIONS FROM ALL ARMY ACTIVITIES, ACTIVE AND RESERVE COMPONENTS. NOMINATIONS SHOULD BE SUBMITTED THROUGH THE CHAIN OF COMMAND TO THE OFFICE OF THE DEPUTY CHIEF OF STAFF, G-1, (ATTN: DAPE-CPS-ES) ARMY INCENTIVE AWARDS BOARD, HOFFMAN BUILDING I, ROOM 476, 2461 EISENHOWER AVENUE, ALEXANDRIA VA 22331-0300 BY 31 JULY 2009, COVERING CALENDAR YEAR 2008.
  2. 2. THE AWARD WAS ESTABLISHED BY THE MILITARY DEPARTMENTS IN 1996 IN HONOR OF ZACHARY AND ELIZABETH FISHER. THE FISHERS HAVE CONTRIBUTED EXTENSIVELY TO THE SUPPORT AND WELFARE OF MEMBERS OF THE ARMED SERVICES. THE AWARD IS INTENDED TO HONOR AND RECOGNIZE A PRIVATE SECTOR INDIVIDUAL OR ORGANIZATION THAT HAS DEMONSTRATED EXCEPTIONAL PATRIOTISM AND HUMANITARIAN CONCERNS FOR MEMBERS OF THE U.S. ARMED FORCES OR THEIR FAMILIES.
  3. 3. THE AWARD RECIPIENT WILL BE SELECTED USING THE FOLLOWING CRITERIA:
    1. EXEMPLIFIES THE FISHERS' PERSONAL QUALITIES OF PATRIOTISM, GENEROSITY, AND SELFLESS DEDICATION TO THE MEMBERS OF THE ARMED FORCES THROUGH AN OUTSTANDING HUMANITARIAN ACT OR SERVICE.
    2. SELFLESSLY CONTRIBUTED AN EXTRAORDINARY AMOUNT OF TIME, TALENT, OR RESOURCES TO BENEFIT MEMBERS OF THE ARMED SERVICES. THE IMPACT SHOULD HAVE BEEN BROAD AND WIDE-RANGING IN SCOPE REGARDLESS OF MONETARY IMPACT.
    3. SIGNIFICANTLY ENHANCED THE QUALITY OF LIFE OF MILITARY MEMBERS OR THEIR FAMILIES THROUGH THE ACT OR SERVICE.
  4. THE PERIOD OF SERVICE FOR THE AWARD IS 1 JAN 08 THROUGH 31 DEC 08, OR A BROADER PERIOD FOR A MULTI-YEAR/LIFETIME CONTRIBUTION.
  5. AWARD ELIGIBILITY REQUIREMENTS ARE AS FOLLOWS:
    1. INDIVIDUALS OR ORGANIZATIONS WHO ARE NOT CURRENTLY EMPLOYED BY DOD AND ITS COMPONENTS, OR WHO WERE NOT EMPLOYED DURING THE PERIOD FOR WHICH THE AWARD IS RECOMMENDED. HOWEVER, A PERIOD OF DOD EMPLOYMENT DOES NOT PRECLUDE CONSIDERATION FOR A MULTI-YEAR/LIFETIME CONTRIBUTION AWARD FOR SERVICE PERFORMED WHILE NOT EMPLOYED BY DOD.
    2. INDIVIDUALS OR ORGANIZATIONS HAVING A PROFIT-MAKING RELATIONSHIP WITH DOD, OR WITH A DOD COMPONENT, MAY BE ELIGIBLE TO RECEIVE THE AWARD IF IT IS SHOWN THAT THE CONTRIBUTION IS SUBSTANTIALLY BEYOND THAT SPECIFIED OR IMPLIED WITHIN THE TERMS OF THE CONTRACT ESTABLISHING THE RELATIONSHIP, AND THE RECOGNITION IS CLEARLY IN THE PUBLIC INTEREST.
  6. NOMINATION PACKETS SHALL INCLUDE THE FOLLOWING:
    1. ENDORSEMENT BY A COMMANDER OF AN ARMY COMMAND, ARMY SERVICE COMPONENT COMMAND, OR A DIRECT REPORTING UNIT OR CIVILIAN EQUIVALENT.
    2. SPECIFIED NOMINATION FORMAT.
    3. ELECTRONIC OR STANDARD PHOTOGRAPH.
    4. SUPPORTING DOCUMENTATION, CERTIFICATES, NEWS ARTICLES, AND PAMPLETES.
    5. ADDITIONAL INFORMATION ON HUMANITARIAN ACT OR SERVICE-USE SHORT BULLETS STATEMENTS ON PLAIN PAPER.
    6. A PROPOSED CITATION THAT INCLUDES SPECIFIC EXAMPLES OF THE ACT OR SERVICE RENDERED.
    7. NAME, ADDRESS, ELECTRONIC MAIL, AND TELEPHONE NUMBER OF THE POINT OF CONTACT FOR THE NOMINATING ORGANIZATION.
  7. SELECTION PROCESS IS AS FOLLOWS: AN HQDA-LEVEL PANEL (COMPOSED OF SES MEMBERS) WILL REVIEW AND PRIORITIZE THE NOMINATIONS, RECOMMEND A CANDIDATE, AND FORWARD THAT FINALIST'S NOMINATION TO A MULTI-DEPARTMENT SELECTION RECOMMENDATION COMMITTEE CONSISTING OF EACH MILITARY DEPARTMENT'S ASSISTANT SECRETARY FOR MANPOWER AND RESERVE AFFAIRS, OR DESIGINATED REPRESENTATIVE.
  8. THE AWARD WILL BE PRESENTED BY THE SECRETARY OF THE MILITARY DEPARTMENT WITH PRESENTATION AUTHORITY AT A SPECIAL CEREMONY PRESENTLY SCHEDULED FOR 2009. DATE AND LOCATION TO BE DETERMINED. THE AWARD CONSISTS OF A LARGE COMMEMORATIVE TROPHY BEARING THE RECIPIENT'S NAME. THE TROPHY IS ON PERMANENT DISPLAY AT THE PENTAGON. THE RECIPIENT ALSO RECEIVES A LARGE MEDALLION WITH PRESENTATION CASE AND A LAPEL PIN.
  9. ARMY REGULATION 672-16, ZACHARY AND ELIZABETH FISHER DISTINGUISHED CIVILIAN HUMANITARIAN AWARD, 15 MAY 98, ESTABLISHES POLICY FOR THE AWARD.
