MEMORANDUM FOR SEE DISTRIBUTION
SUBJECT: Retroactive Reimbursement of Federal Employees Health Benefit (FEHB) Premiums for Employees Called to Active Military Duty



  1. Defense Finance and Accounting Service memorandum, dated February 1, 2005, subject, Claim for Retroactive Reimbursement for Federal Employees Health Benefits (FEHB) Premiums While on Active Duty in Support of a Contingency Operation, outlines policy and procedures for claims (Enclosed).
  2. The enclosed "CPMS FAS Sample Form" replaces the previous edition dated May 10, 2004. This version of the form incorporates the following:
    1. It is no longer necessary to report the major claimant code (from Block 47 of the SF 50) in effect during the period of service on which the claim is based.
    2. FEHB premiums for current employees will be charged to the funds of the claimant's current employing agency---regardless of the employing activity at the time of mobilization.
    3. Claims for employees not currently employed, for instance, separated or retired claimants, must include a Line of Accounting (LOA) on the form. The LOA will be required prior to submitting claims to the payroll office. The Army's single accounting classification for the payment of all Army's retro FEHB claims under PL 107-107, Section 519 is below.
